Post the Patient Prism Code Snippet on Your Website π»
To track new form submissions, you'll first need to post your Patient Prism code on your website:
Go to the Patient Prism Integrations Page: Manage β Integrations.

Insert the code snippet between the
<head>and</head>tags.If the code is already on the site because you are using a dynamic number, the code does not need to be placed again.
Enable the Integration βοΈ
Now that the code has been placed, enable the integration:
1. Go to the Patient Prism Integrations Page: Manage β Integrations.
2. Go to the Form Tracking section and enable it by clicking the radio button:
Grey = Disabled
Blue = Enabled
3. Press Save.
4. Use the Blacklisted Form IDs feature to prevent specific form IDs on your website from tracking submissions. Enter the URL of the form page that you do not want to be tracked.
Assign the Form to a Location π
To have the form automatically assigned to a location, you can enter the location website URL in the location settings page:
