You can send your form submissions to Patient Prism to manage them and follow up with the potencial leads:
Full Forms Integration Section
To track new form submissions, you'll first need to post your Patient Prism code on your website.
- Go to the Patient Prism Integrations Page: Manage --> Integrations

Insert the code snippet between the
<head>
</head>
tags.
* If the code is already on the site because you are using a dynamic number, the code does not need to be placed again
Now that the code is placed enable the Integration.
- Go to the Patient Prism Integrations Page: Manage --> Integrations
- Go to the Form Tracking Section and enable it by clicking the radio button
Grey = Disabled
Blue = Enabled
- Finish by pressing the Save button.
- You can use the Blacklisted Form IDs feature to prevent specific form ids on your website from tracking submissions.
You can enter the URL of a form page that you do not want to be tracked.
We will now be able to track form data in our system.
To have the form automatically assigned to a location, you can enter the location website URL in the location settings page:
- Go to Manage > Locations and select the location you would like to edit.
- Press the edit button on the top right:
- Press the + Add Website button:
- Type in your website URL and then press Save:
- The next time we capture a form submission from that website, we will automatically associate it with the location where the URL was saved.
