How to Setup the Forms Integration

First, You must place your Patient Prism code snippet on your website.

  1. Go to the Patient Prism Integrations Page: Manage --> Integrations

  2. Insert the code snippet between the <head>  </head> tags.
    * If the code is already on the site because you are using a dynamic number, the code does not need to be placed again

 

Now that the code is placed enable the Integration.

  1. Go to the Patient Prism Integrations Page: Manage --> Integrations
  2. Go to the Form Tracking Section and enable it by clicking the radio button

  3. Finish by pressing the Save button.
  4. You can use the Blacklisted Form IDs feature to prevent specific form ids on your website from tracking submissions.

    You can enter a URL of a form page that you do not want to be tracked.

We will now be able to track form data in our system.
