What Does the Forms Integration Do?

Using the Patient Prism Forms integration, you can access your website form data in the Patient Prism system.

By placing our code snippet on your website and enabling the integration, the next time a patient submits a form, it will generate data within Patient Prism.

This will allow you to easily access the form data in Patient Prism and track phone leads and form leads in one system.

You can also place notes on the forms in Patient Prism and indicate if the lead booked an appointment or not via the follow up process.

We will explain on the next page how to set this up, but it consists of two major steps:

1. Adding the Patient Prism code snippet to your website
2. Enabling Submission Tracking
